Record Application Form for Recording Catches

Read all MLF SA Record Requirements before completing and signing this application. The angler’s signature on the completed form must be witnessed by a notary. This application must be accompanied by line or tippet samples and photographs as specified in the Record Requirements. Hybrids and other species that may pose a problem of identity should be examined by an ichthyologist or qualified fishery biologist.
